OBJECTIVE: The aim of this study was to evaluate the Latin American orthopaedic trauma surgeons preference regarding knee positioning and entry portals for IM nailing and identify the reasons of these preferences. METHODS: Using the AO Trauma database, 22.285 surveys were distributed by email to Latin American orthopaedic surgeons. Demographic data and practice patterns, especially regarding knee positioning and approach for tibial nailing, were then evaluated and statistically treated. RESULTS: amongst one thousand five hundred fourteen responses, 990 orthopaedic surgeons (4.4% of response rate) fully responded to the survey. Transpatellar tendon approach (613 / 61.9%,) with the knee in flexion (518 / 52.3%) on a radiolucent table remains the standard practice for intramedullary tibial nailing. Even for proximal and distal tibial nailing, the transpatellar tendon approach (455 / 46%) with the knee in flexion (562 / 56.8%) hold on the most used method. Only 55 (9.36%) orthopaedic surgeons reported that they have migrated to the supra-patellar tibial nailing in the recent years. The main reasons for a low rate of migration were lack of knowledge about the technique and unavailability of specific gigs and cartilage protectors for a safe suprapatellar nailing. CONCLUSION: Even with the potential benefits of the semi-extended knee positioning for tibial nailing, the Latin American orthopaedic community remains using the transpatellar tendon approach with the knee in flexion as the standard technique. Lack of surgical training for suprapatellar and parapatellar approaches with the knee in semi-extension, added by the unavailability of suprapatellar jigs and soft outer protection sleeves contribute to counter the trend towards the semi-extended techniques.

INTRODUCTION: Isolated fractures of the iliac wing are a rare injury, usually occurring in high-energy trauma, and are associated with other non-musculoskeletal and soft tissue injuries that could compromise the patient's life. Surgical indications are unclear, and there is limited information on the most frequent fracture patterns. METHODS: A descriptive multicenter case series study of isolated fractures of the iliac wing treated surgically in three referral trauma hospitals in Latin America. The different fracture patterns are described, the "iliac ring" concept is proposed, and a classification is made. RESULTS: Twenty-eight patients were included; 24 were male, the median age was 31 years RIQ (24-46), the most frequent trauma mechanism was a traffic accident, and in 14 patients, the ISS >16. The most frequent associated injury was to the appendicular skeleton at another level in 13 patients. In the new classification, according to the number of fragments, 11 patients were classified as type A (1 fragment), ten patients as type B (two fragments), and seven patients as type C (three or more fragments). The most compromised anatomical area was the crest and anterosuperior iliac spines in 26 patients, followed by the fossa and anteroinferior iliac spine in 17 and 8 patients, respectively. DISCUSSION: The patterns of isolated fractures of the iliac wing allow the identification of three types of fractures. Identifying these patterns can help the surgeon decide to perform surgery in these scenarios and choose the fixation technique according to the number and location of the fragments.

Determining the true availability of resources and understanding the level of training of surgeons involved in the treatment of patients with pelvic fractures and haemorrhagic shock is critical. In the herein study, the availability of technical, technological, and human resources for the care of this injury in Latin America region was analysed, and the preferences of orthopaedic trauma surgeons when performing interventions for the diagnosis and treatment of patients with pelvic trauma and associated haemorrhagic shock was described. A cross sectional web-based survey containing questions on knowledge, attitudes, and practices with respect to imaging resources, emergency pelvic stabilization methods, and interventions used for bleeding control was sent to 948 Latin America orthopaedic trauma surgeons treating pelvic fractures in the emergency department. Differences between regional clusters, level of training, type of hospital, and pelvic surgery volume were assessed. 368 responses were obtained, with 37.5% of respondents reporting formal training in pelvic surgery and 36.0% having available protocol for managing these patients. The most frequently used interventions were the supra-acetabular pelvic external fixator and pelvic packing. Limited hospital and imaging resources are available for the care of patients with pelvic trauma and associated haemorrhagic shock throughout Latin America. In addition, the training of orthopaedic trauma surgeons dealing with this type of injury and the volume of pelvic surgeries per year is heterogeneous. It should be urgently considered to develop management protocols adapted to Latin America according to the availability of resources, as well as to promote training in this severe life-threatening traumatic condition.

RESUMEN

Postoperative bone infection is a severe complication in the treatment of fractures. The management of this pathology is challenging, but recent advances have been made to achieve standardization that can help diagnosis and decision-making. However, we are unaware of studies validating these models in Latin America. Therefore, this study aims to collect data from patients with fracture-related infections treated in different institutions in Latin America to create a registry that will assist in future clinical decision-making regarding the diagnostic process and the surgical and medical treatment of these patients.

RESUMEN

The widespread adoption of smartphones and other mobile devices amongst healthcare providers opened new possibilities arising from the use of non-medical apps, social media, meeting platforms, and non-medical devices with intended medical purposes, thus expanding the communication and imaging chat systems between these professionals and their patients, as well as amongst healthcare professionals. However, adapting non-medical applications, social media, videoconference platforms and devices for medical use present potential limitations, barriers, and risks, which should be fully recognized to reduce crossing the fine line between ethical and unethical. In the herein study, we analyse the ethical limits, coverage, and validation of non-medical applications adapted for medical use. Level of evidence: IV (evidence from well-designed case-control or cohort studies).

RESUMEN

Fracture-related infection (FRI) is a complication that impacts care costs, quality of life, and patient function. Great strides have been made in the last decade to obtain a common language for definition and diagnosis with the contribution of the Fracture-Related Infection Consensus. Although FRI treatment requires the participation of clinical specialists in infectious diseases for the management of antibiotics, it is necessary to understand that this complication is an eminently surgical pathology. The orthopedic surgeon must play a leadership role in the prevention and treatment of this complex disease. In this review, the most relevant aspects of prevention are updated, and a strategy for a sequential and comprehensive approach to the patient with this complication is presented.

This study aims to evaluate the effectiveness of an online course to enable orthopaedic surgeons to acquire the core competencies necessary to prevent and treat fracture-related infections (FRI). This study included orthopaedic surgeons and residents from Latin American countries who attended an online course focused on FRI. The online course included: didactic lectures, small-group clinical case discussions, and panel case discussions. The course was delivered using Zoom® platform and designed to address four core competencies: prevention, definition and diagnosis, antimicrobial therapy, and surgical treatment. An online questionnaire was created distributing 16 questions through six clinical scenarios. Participants were invited to answer the questionnaire before and after the course. Sixty of the 78 course participants answered the pre-course, and 42 the post-course assessment. Relative to before the course, the mean post-course assessment score rose significantly for prevention of FRI (4.1 before and 4.5 after; p = 0.014), definition and diagnosis (2.4 before and 3.4 after; p = 0.001), and surgical treatment (2.2 before and 2.8 after; p = 0.011). The final score encompassing all four core competencies also rose significantly (2.7 before and 3.3 after; p = 0.001). The online course on FRI was feasible and effective, significantly increasing course users' knowledge of overall competency in managing FRI.

Resumen La infección del muñón después de amputaciones traumáticas tiene una prevalencia hasta del 34%. Las bacterias más frecuentemente aisladas son Staphylococcus aureus, Enterobacterales como Escherichia coli; Pseudomonas aeruginosa, entre otras. Estas infecciones ocurren por la inoculación directa en el momento del trauma o por gérmenes nosocomiales; la realización de curaciones de las heridas con emplastos vegetales es una práctica aún frecuente en zonas rurales de nuestro país pero su relación con infección del sitio operatorio ha sido poco explorada en la literatura. Leuconostoc spp. es un coco Gram positivo encontrado en territorio agrícola y utilizado en la industria de alimentos. Se presenta un caso de infección de un muñón transfemoral por Leuconostoc, después de una amputación traumática del miembro inferior en una paciente previamente sana con una posible asociación a curaciones con emplastos vegetales.


Abstract Infection of a traumatic amputation stump has a prevalence of 34%. The most common bacteria isolated are Staphylococcus aureus, Pseudomonas aeruginosa and Enterobacterales such as Escherichia coli. These infections occur by direct inoculation in the moment of the trauma or by nosocomial germs. Infections secondary to manipulation of the wounds with vegetable plasters have few case reports in the literature. Leuconostoc spp. is a Gram-positive coccobacillus commonly found in agricultural territory and used in the food industry. There are few case reports in the literature about bone infections by Leuconostocs pp. We present a case of an infection of the operative site of a transfemoral stump by Leuconostoc spp. after a traumatic amputation of the lower limb in a previously healthy patient who had a possible association to cures with vegetable plasters.

Introducción El pie diabético infectado es una complicación frecuente de la diabetes y un marcador de deterioro del paciente. Existe escasa información en Colombia sobre características de los pacientes que ingresan a los servicios de urgencias de alta complejidad con esta patología y desenlaces como amputación y perfil microbiológico. Materiales y Métodos Serie de casos de pacientes que ingresaron al servicio de urgencias del Hospital Pablo Tobón Uribe con motivo de consulta principal pie diabético infectado. Se analizaron las historias clínicas de manera retrospectiva y se utilizaron herramientas de estadística descriptiva para la caracterización de la población y de variables relacionadas con diagnóstico, tratamiento y resultados tempranos intrahospitalarios. Resultados Entre enero de 2009 y diciembre de 2013 ingresaron 118 pacientes con 145 úlceras infectadas por pie diabético, el 90% con HbAc>6.5, el 52% con disfunción renal, el 51% con enfermedad arterial periférica. El 57% tenía úlceras grado 3 o mayor en la clasificación de Wagner, la infección fue polimicrobiana en el 63%. El 58% requirió amputación, el 62% de las amputaciones fueron amputaciones menores. La mortalidad fue del 10%, la mortalidad atribuible a infección del pie diabético o complicaciones derivadas de la amputación fue del 1,7%. Discusión Los pacientes con pie diabético que ingresaron a urgencias de un hospital de alta complejidad en Colombia tienen una enfermedad sistémica avanzada, relacionada con disfunción renal y vascular periférica, lesiones locales graves con compromiso óseo y articular avanzado; dada la gravedad de estas condiciones, la prevalencia de amputaciones mayores puede ser superior a la reportada en la literatura. Background Infected diabetic foot is a frequent complication of diabetes and a marker of patient deterioration. There is little information in Colombia on the characteristics of patients that enter the highly complex emergency services with this condition and their outcomes, such as amputation and microbiological profile. Materials Case series of patients admitted to the emergency department of Pablo Tobón Uribe Hospital whose main consultation was infected diabetic foot. The medical records were retrospectively analysed and descriptive statistical tools were used to characterise the population, as well as the variables related to diagnosis, treatment, and early in-hospital outcomes. Results Between January 2009 and December 2013, 118 patients with 145 ulcers due to an infected diabetic foot were admitted. The HbA1c was> 6.5 in 90%, and 52% had renal dysfunction, 51% with peripheral arterial disease, and 57% had ulcers grade 3 or higher in the Wagner classification. The infection was polymicrobial in 63%, and 58% required amputation, with 62% of amputations being classed as minor. Mortality was 10%, and mortality attributable to diabetic foot infection or complications derived from amputation was 1.7%. Discussion Patients with diabetic foot admitted to the emergency room of a high complexity hospital in Colombia have an advanced systemic disease, related to renal and peripheral vascular dysfunction, and serious local injuries with advanced bone and joint involvement. Given the severity of these conditions, the prevalence of major amputations may be higher than that reported in the literature. Evidence Level: IV

BACKGROUND: Traffic accidents are considered a public health problem and, according to the World Health Organization, currently is the eighth cause of death in the world. Specifically, pedestrians, cyclists and motorcyclists contribute half of the fatalities. Adequate clinical management in accordance with aggregation patterns of the body areas involved, as well as the characteristics of the accident, will help to reduce mortality and disability in this population. METHODS: Secondary data analysis of a cohort of patients involved in traffic accidents and admitted to the emergency room (ER) of a high complexity hospital in Medellín, Colombia. They were over 15 years of age, had two or more injuries in different areas of the body and had a hospital stay of more than 24 h after admission. A cluster analysis was performed, using Ward's method and the linfinity similarity measure, to obtain clusters of body areas most commonly affected depending on the type of vehicle and the type of victim. RESULTS: Among 2445 patients with traffic accidents, 34% (n = 836) were admitted into the Intensive Care Unit (ICU) and the overall hospital mortality rate was 8% (n = 201). More than 50% of the patients were motorcycle riders but mortality was higher in pedestrian-car accidents (16%, n = 34). The clusters show efficient performance to separate the population depending on the severity of their injuries. Pedestrians had the highest mortality after having accidents with cars and they also had the highest number of body parts clustered, mainly on head and abdomen areas. CONCLUSIONS: Exploring the cluster patterns of injuries and body areas affected in traffic accidents allow to establish anatomical groups defined by the type of accident and the type of vehicle. This classification system will accelerate and prioritize ER-care for these population groups, helping to provide better health care services and to rationalize available resources.

RESUMEN

Hemicorporectomy is an ultra-radical surgery used only in extreme circumstances. Initially used for advanced pelvic neoplastic diseases and intractable pelvic infection, it may also be the only treatment option in patients with crushed pelvic trauma, in cases there are no reconstruction options. This procedure has a high mortality, and its success depends on the multidisciplinary approach, both in the initial phase and in the rehabilitation process. We present the case of a young patient with severe pelvic trauma that required a hemicorporectomy as the only treatment option and review of the literature.

La uretroplastia transpúbica es una técnica empleada para tratar la estenosis de uretra proximal compleja, permitiendo una mejor visualización del campo quirúrgico y una mayor facilidad al momento de realizar la anastomosis. Existen pocas descripciones en la literatura de los resultados quirúrgicos y funcionales en los pacientes sometidos a ese procedimiento, el propósito de este estudio es describir una serie de casos complejos tratados con ese abordaje, de una manera interdisciplinaria entre Urología y Ortopedia


Transpubic urethroplasty is a technique used to treat complex proximal urethral stricture, allowing better visualization of the surgical field and greater ease at the time of performing the anastomosis. There are few descriptions in the literature of the surgical and functional results in the patients undergoing this procedure, the purpose of this study is to describe a series of complex cases treated with this approach, in an interdisciplinary way between Urology and Orthopedics.

Abstract Introduction: Rapid recovery and low cost are among the benefits of ketamine for emergency sedation. It has been excluded as the first choice because of the associated adverse events. Objective: To describe the adverse events associated with the use of ketamine in a high-complexity emergency service. Materials and methods: Review of clinical records of patients who received sedation with ketamine for orthopedic procedures in the emergency room between January 2012 and June 2015, with identification of adverse events. Results: Overall, 354 patients were identified (74% males, 32% children), with a median age of 21 years, interquartile range (IQR) of 20 years. Of them, 66% had upper limb injuries, 79% were treated on an outpatient basis, with a median length of stay in the emergency service of 3.6hours (IQR 2,5). In 98%, sedation was given by a different practitioner from the orthopedic surgeon. Ketamine and midazolam were administered together in the same proportion, and 3 or more medications were used in 13% of cases. Overall, 14 adverse events (3.9%) were described, 9 related to desaturation between 80% and 90% which was solved with oxygen through nasal cannula, 3 were cases of vomiting following sedation with no aspiration, and 2 were cases of desaturation <80% which were managed with oxygen administration through a cannula and maneuvers to maintain airway patency. One patient had visual hallucinations. No patient required advanced airway maneuvers. Conclusion: The use of ketamine for sedation in the emergency service is associated with a low prevalence of major adverse events. Sedation with ketamine and midazolam appears to be a safe strategy for these procedures.


Resumen Introducción: La ketamina para sedación en urgencias tiene beneficios tales como recuperación rápida y bajo costo. Se ha excluido como primera opción por sus eventos adversos. Objetivo: Describir los eventos adversos relacionados con el uso de ketamina en un servicio de urgencias de alta complejidad. Materiales y métodos: Se revisaron historias clínicas de pacientes que recibieron ketamina en urgencias para sedación en procedimientos ortopédicos entre enero de 2012 y junio de 2015, identificando eventos adversos. Resultados: Se identificaron 354 pacientes, (74% hombres, 32% niños), con una mediana de edad de 21 años, rango intercuartílico (RIQ) de 20 años. 66% con lesiones del miembro superior, 79% tratados ambulatoriamente con una mediana de estancia en urgencias de 3,6 horas (RIQ 2,5). En el 98% la sedación fue realizada por un médico diferente al ortopedista. En la misma proporción se administró ketamina y midazolam conjuntamente, en 13% se utilizaron tres o más medicamentos. Se describieron 14 eventos adversos (3,9%), nueve correspondían a desaturación entre 80 y 90% resuelta con oxígeno por cánula nasal, tres casos de vómito después de la sedación sin broncoaspiración, y dos casos de desaturación <80% resuelta con oxígeno por cánula y maniobras para permeabilización de la vía aérea. Hubo alucinaciones visuales en un paciente. Ning un paciente requirió maniobras avanzadas para la vía aérea. Conclusiones: El uso de ketamina para sedación en urgencias tiene una baja prevalencia de eventos adversos mayores. La sedación con ketamina y midazolam, parece ser una estrategia segura en estos procedimientos.

BACKGROUND: Our purpose was to validate the performance of the ISS, NISS, RTS and TRISS scales as predictors of mortality in a population of trauma patients in a Latin American setting. MATERIALS AND METHODS: Subjects older than 15 years with diagnosis of trauma, lesions in two or more body areas according to the AIS and whose initial attention was at the hospital in the first 24 h were included. The main outcome was inpatient mortality. Secondary outcomes were admission to the intensive care unit, requirement of mechanical ventilation and length of stay. A logistic regression model for hospital mortality was fitted with each of the scales as an independent variable, and its predictive accuracy was evaluated through discrimination and calibration statistics. RESULTS: Between January 2007 and July 2015, 4085 subjects were enrolled in the study. 84.2% (n = 3442) were male, the mean age was 36 years (SD = 16), and the most common trauma mechanism was blunt type (80.1%; n = 3273). The medians of ISS, NISS, TRISS and RTS were: 14 (IQR = 10-21), 17 (IQR = 11-27), 4.21 (IQR = 2.95-5.05) and 7.84 (IQR = 6.90-7.84), respectively. Mortality was 9.3%, and the discrimination for ISS, NISS, TRISS and RTS was: AUC 0.85, 0.89, 0.86 and 0.92, respectively. No one scale had appropriate calibration. CONCLUSION: Determining the severity of trauma is an essential tool to guide treatment and establish the necessary resources for attention. In a Colombian population from a capital city, trauma scales have adequate performance for the prediction of mortality in patients with trauma.

Introduction: Throughout the years, several methods have been developed to help determine injury severity and obtain accurate prognoses in trauma patients. Trauma scores that have been used for more than 40 years are extremely useful in clinical practice as well as in research. Objective: To conduct a review of the most relevant literature on trauma and to make a description of each of the scoring tools, focusing on their limitations and their application in clinical trials. Materials and methods: Narrative review conducted in different databases such as PubMed, ScienceDirect and OVID. A manual search was also conducted of articles on the subject in both English and Spanish. Results: The review articles provided an adequate description of each of the scores, the way they are calculated, the main limitations in their application, and the most relevant findings in the literature. Conclusion: There is a wide range of severity scores used in trauma patients for anticipating clinically significant outcomes with varying degrees of accuracy. Creating and validating a single, universally valid score is a huge challenge; consequently, the selection of the scoring tool is based, to a large extent, on experience, the context and the available evidence.


Introducción: a lo largo de la historia se han creado varios métodos para evaluar la gravedad de las lesiones y brindar un pronóstico exacto en pacientes con trauma. Los puntajes en trauma que se han utilizado por más de 40 años son una herramienta de gran utilidad tanto para el contexto clínico como investigativo. Objetivo: elaborar una revisión de la literatura más relevante sobre los puntajes en trauma y hacer una descripción de cada una de estas herramientas, haciendo énfasis en sus limitaciones y en la aplicación en estudios clínicos. Materiales y Métodos: revisión narrativa, se consultaron diferentes bases de datos como PubMed, ScienceDirect y OVID; además, se hizo búsqueda manual de artículos en inglés y en español sobre el tema. Resultados: los artículos revisados permitieron hacer una descripción adecuada de cada uno de los puntajes, de la forma en que se calculan, sus principales limitaciones al momento de aplicarlos y los hallazgos más notables en la literatura. Conclusión: existe una gran variedad de puntajes de gravedad para pacientes con trauma que permiten anticipar con diferente exactitud los desenlaces clínicamente significativos. La creación y validación de un único puntaje universalmente válido es todo un reto; por ello la selección de esta herramienta está basada en gran parte en la experiencia, el contexto y la evidencia disponible.

Background: The soft tissues injury in periarticular fractures of the lower extremities determines the proper time to perform bone fixation. Objective: The aim of this study was to determine the intra and interobserver agreement in the Tscherne classification. Methods: This is a descriptive, prospective study for patients admitted to the Pablo Tobón Uribe Hospital (PTUH) with tibial plateau or tibial pilon fractures. We performed a standardize evaluation using video photography at the time of admission and 24, 48, and 72 h after admission. Fifteen five reviewers who had various levels of training produced a total of 1,200 observations. The intra- and interobserver agreement was assessed using a weighted kappa for multiple raters and more than two categories. Results: Twenty patients were admitted with tibial plateau and tibial pilon fractures. The intraobserver agreement for all 15 raters was kappa 0.81 (95% CI 0.79-0.83), and the interobserver agreement for all 15 raters was kappa 0.65 (95% CI 0.55-0.73). The interobserver agreement at 24 h was kappa 0.67 (95% CI 0.46-0.86). Conclusions: Classifying the severity of soft tissue injury is critical in planning the surgical management of fractures of the lower extremities. Based on our results, we can reasonably argue that the Tscherne classification produced an adequate level of agreement and could be used to standardize and to guide the treatment, and to conduct research studies.


Antecedentes: La lesión de los tejidos blandos en fracturas periarticulares del miembro inferior determina el momento adecuado para realizar la fijación ósea. Objetivo: El propósito de este estudio fue determinar la concordancia intra e interobservador para la clasificación de Tscherne. Métodos: Estudio descriptivo, prospectivo en pacientes ingresados al Hospital Pablo Tobón Uribe con fracturas de platillos tibiales o pilón tibial. Se realizó una evaluación estandarizada utilizando videos y fotografías al momento del ingreso y 24, 48 y 72 h después. Quince evaluadores que tenían diferentes niveles de entrenamiento produjeron 1200 observaciones. La concordancia intre e interobservador fue evaluada utilizando kappa ponderado para múltiples evaluadores y más de dos categorías. Resultados: Veinte pacientes fueron admitidos con fracturas de los platillos o del pilón tibial. La concordancia intraobservador para los 15 evaluadores fue kappa 0.81 (95% IC 0.79-0.83), y la concordancia interobservador para los 15 evaluadores fue kappa 0.65 (95% IC 0.55-0.73). La concordancia interobservador a las 24 h del ingreso fue kappa 0.67 (95% IC 0.46-0.86). Conclusión: Clasificar la gravedad de la lesión de los tejidos blandos es un paso fundamental en la planeación del manejo quirúrgico de las fracturas periarticulares del miembro inferior, con base en nuestros resultados podemos afirmar razonablemente que la clasificación de Oestern y Tscherne tiene un nivel de concordancia adecuado y podría utilizarse de forma sistemática para estandarizar la evaluación entre los ortopedistas para orientar el tratamiento y también con fines investigativos.

Objetivo general: Describir la epidemiología del Síndrome de Dificultad Respiratoria Aguda (SDRA) postraumático. Metodología: Estudio observacional descriptivo de pacientes con fractura traumática de la diáfisis femoral que requirieron manejo quirúrgico. Se analizaron variables demográficas, gravedad del trauma, métodos de tratamiento y desenlaces intrahospitalarios. Se realizó un análisis descriptivo y multivariado para explorar factores asociados al desarrollo de SDRA. Resultados: Doscientos sesenta y siete pacientes con fractura femoral, el 86% fueron hombres con edad media de 28,7 años e Índice de Severidad del Trauma (ISS) de 12,7. Se observó una prevalencia de SDRA del 8,6% en toda la población y del 20% en politraumatizados. La mortalidad fue del 7,5%. Las variables asociadas al SDRA en el análisis bivariado fueron: tiempo entre el trauma y el ingreso, ISS, contusión pulmonar, fractura asociada de tibia, requerimiento de cirugía urgente y necesidad de transfusión de glóbulos rojos. En el análisis multivariado, las variables asociadas fueron: trauma contuso de tórax, tiempo transcurrido hasta la fijación definitiva y la transfusión de glóbulos rojos en cirugía. En la cohorte hubo un incremento en la prevalencia de SDRA anual, siendo del 4,3% en 2006 comparado con el 26,1% en 2011, al igual que el politrauma que pasó del 14,5% en 2006 al 23,6% en 2011. Se evidenció también un cambio en el tratamiento, aumentando el control temprano total y el control de daño ortopédico. Conclusiones: En nuestra población, la prevalencia de SDRA en pacientes con fracturas de fémur aumenta cuando hay trauma contuso de tórax, cuando se prolonga el tiempo de fijación y la transfusión sanguínea.


Objective: To describe the epidemiology of post-traumatic acute respiratory distress syndrome. Methodology: Descriptive observational study of patients with traumatic femoral shaft fracture requiring surgical management. The variables included in the analysis were demographics, injury severity, treatment methods, and in-hospital outcomes. A descriptive multivariate analysis was performed in order to explore the factors associated with the development of Acute Respiratory Distress Syndrome (ARDS). Results: Of the 267 patients with femoral fractures, 86% were male patients with a mean age of 28.7 years and Injury Severity Score (ISS) of 12.7. The overall prevalence of ARDS was 8.6% while the prevalence among multiple trauma patients was 20%. Mortality was 7.5%. In the bivariate analysis, the variables associated with ARDS were the following: time between the trauma and hospital admission, ISS, contused lung, associated tibial fracture, urgent surgery requirement, and need for red blood cell transfusion. In the multivariate analysis, the associated variables were: blunt chest trauma, time elapsed until definitive fixation, and red blood celltrans fusion during surgery. In the cohort, there was an increase in the annual prevalence of ARDS, from 4.3% in 2006 to 26.1% in 2011, as was also the case with multiple trauma, which increased from 14.5% in 2006 to 23.6% in 2011. A change in treatment was also evidenced, with increase in early total care (ETC) and damage control orthopaedics (DCO). Conclusions: In our population, the prevalence of ARDS in patients with femoral fractures increases when associated with blunt chest trauma, delayed stabilization time, and the need for blood transfusion.

RESUMEN

El propósito de este estudio es mostrar nuestra experiencia con clavos flexibles de acero en el tratamiento de las fracturas diafisiarias del fémur en niños entre 5 y 12 años y observar si el material de los clavos altera los resultados. Se hizó un estudio prospectivo tipo serie de casos evaluando los pacientes intervenidos en el HUSVP que hubieran completado un seguimiento mínimo de 24 meses. Se obtuvieron 48 pacientes con un promedio de edad de 8.6 años. El tipo de fractura según la clasificación de la AO más frecuente fue 32A3.2 (61%). El seguimiento promedio fue de 43 meses. Se observaron 6 casos de discrepancia de longitud clínica, dos casos de sobrecrecimiento, cuatro casos de acortamiento y dos mal uniones con deformidad en valgo, sin que estas alteraciones repercutieran en la función del niño. La principal fortaleza de este reporte se encuentra en su seguimiento y nos muestra que, a dos años, los resultados con clavos de acero son tan buenos como los reportados con clavos de titanio con seguimientos más cortos. Estos resultados son relevantes para un medio como el nuestro donde los costos suelen ser prohibitivos.
